Holistic Medicine: The Continuum of Care

This presentation reviews the current challenges of caring for an influx of aging Boomers - most with chronic conditions - and how creative approaches to care delivery will be vital with our shortage of care providers. The presentation shares how leveraging technology, designing alternative care environments, and supporting integrated models of care will be crucial to meeting these challenges. Flexible and adaptable design concepts for outpatient environments that support an integrated and coordinated continuum of care team will be discussed.
